Scope of seminar

Today, it is widely recognised that learning from experiences is one of the main pillars of modern safety approaches. Organisations and society at large can identify valuable lessons from the analysis of safety investigations of events, near-misses, incidents and accidents.

However, there is often a gap between the conclusions of event analysis and consequent safety recommendations, and implementation of changes at all the levels which can contribute to safety improvement (industrial operating companies, competent authorities and regulators, the judiciary, lawmakers, and even society in general). Some lessons are not learned well and some are forgotten by people and organisations. These may be considered as learning deficiencies, which have been found to be contributing factors to several major accidents.

This raises several key questions:

• What are the obstacles to learning and following up on safety recommendations at the different societal levels concerned?
• Does learning from incidents and accidents improve safety performance sufficiently?
• Can unwanted events be prevented through learning?
• How can organizations learn to learn? What are the implications?
• How to turn learning into dynamic learning? How can organizations detect the need to unlearn lessons which are no longer appropriate?
• How to build and maintain a collective memory of lessons and build-up new knowledge?
• Which guidance on learning could be provided to people and organisations?
• How to follow-up and monitor the implementation of recommendations?
• Who has the responsibility for learning? Does the legal framework provide the right incentives to each of the parties involved?
• Can we identify new competencies which are important for health and safety professionals, and further needs in the training of people?
• Can we test the quality of organisational, societal and multi-level learning?
• Could accident investigations be changed to achieve better safety learning?

 

The 45^th ESReDA seminar will be a forum for exploring these questions. We aim to discuss theories, concepts, and experiences with dynamic learning from incidents and accidents, present cases of failure and success from implementation in various sectors and to identify future needs in safety research and training. In relation to the main theme, we especially want to encourage new ideas, scientific papers, conceptual papers, case studies and cross-sectorial research. This seminar will bring together researchers on learning processes, investigators, specialists on safety analysis, decisions makers in the field of learning from experience and accident investigation, health and safety risk managers and training specialists, to discuss the strategies to bridge the gap between safety recommendations and learning.

It builds on the 2003 24^th ESReDA Seminar at the JRC-Institute for Energy in Petten (the Netherlands) on ‘Safety investigation of accidents’; the 33^rd ESReDA Seminar on ‘Future challenges of Accident investigations’ at the JRC-Institute for the Protection and Security of the Citizen in Ispra (Italy); and on the 36^th ESReDA seminar on ‘Lessons learned from accident investigations’ at EDP in Coimbra (Portugal).

Target groups and domains of application (examples)

Papers for the seminar are invited from various stakeholders (industrialists, regulators, safety boards, universities, R&D organisations, engineering contractors and consultants, training specialists) and could address different sectors:

• Energy sector: nuclear and non-nuclear (e.g. fossil, hydro) power plants and networks
• Process industry: oil and gas, chemical and petrochemical facilities
• Transport (rail, road, air and maritime): supply and distribution network, operation
• Aerospace industry
• Critical infrastructure: electricity, water, telecommunications, information systems
• Public sector and government.

 

This seminar is aimed at learning from accident investigation on an organisational or societal level and does not focus on individual learning practices. Other topics may be included if they fit well within the theme of the seminar: natural disasters, na-tech disasters, food safety, sanitary crisis, and banking.

For the fifth consecutive year, EDP is deemed on of the electrical companies with better performance on the Dow Jones Sustainability Indexes. About Porto

It was in Porto and the north that Portugal came into being as a country, which is why the region has such a rich and fascinating heritage. This is a mountainous area with rivers, natural parks and steep, sloping hillsides covered with leafy vegetation. The granite from its mountains was used to build many of the region’s religious and historical monuments, such as the unpretentious Romanesque chapels and baroque churches.

The fact remains that this ancient city gave its name to both Port wine and to the nation of Portugal. Much of its heritage is based on shipping and trade, making it a bustling and passionate city. Porto is also classified as world heritage site. Perhaps these are the reasons why Porto finds its way into the hearts of its visitors.

European Safety, Reliability & Data Association (ESReDA) is an European Association established in 1992 to promote research, application and training in Reliability, Availability, Maintainability and Safety (RAMS). The Association provides a forum for the exchange of information, data and current research in Safety and Reliability.

ESReDA membership is open to organisations, privates or governmental institutes, industry researchers and consultants, who are active in the field of Safety and Reliability. ESReDA Project Group on Dynamic learning as the follow-up from accident investigations

This ESReDA project group was founded in 2008 to work on how to disseminate results obtained in event investigations of high-risk events (accident and near-misses). The project group has organised the 36th ESReDA Seminar “Lessons learned from accident investigations” at EDP in Coimbra in 2009. This Project Group is continuing from the work done by the ESReDA Working Group on Accident Investigation that has organised two seminars:

• the 33rd ESReDA Seminar on “Future challenges of accident investigation”;
• the 24th ESReDA Seminar on “Safety Investigation of Accidents”.

It has prepared three deliverables which have been printed and published by ESReDA:

• Accident Investigation Practices – Results from a European Study (2003 – report);
• Shaping Public Safety Investigations of Accidents in Europe (2005 – ESReDA Safety Series);
• Guidelines for safety investigation of accidents (2008) available on the ESReDA website.
